  • Bilingual Pay/Test Request

    To determine whether an employee is able to receive bilingual pay, the department must have the employee tested. Use this form to request that the test be scheduled.

  • Alternate Work Schedule Request

    This form is for County employees to request a change in their work schedule.

  • Work Out Of Class

    WOOC is a temporary assignment of additional higher level duties to be performed during the absence of an employee in a higher classification.

  • WOOC Assignment to Vacant Code Approval

    This form is to be used by the Department and submitted to Human Resources prior to assignment.

  • Promotional Rating (General)

    Promotional Rating Forms are used when promoting a current County employee in their own position.

  • Ingrade Appointment Request
    An Ingrade Appointment allows the County to appoint a newly hired employee to a step higher than the lowest step listed in the Basic Salary Plan. To appoint an employee to step two, three, four or five, the department head must prepare a transmittal and the "Ingrade Appointment Request Form".
  • Clinical Nurse and Psychiatric Nurse Salary Placement Form
    The department must complete the Clinical Nurse (CN) and Psychiatric Nurse (Psych Nurse) Salary Placement Form for all CN and Psych Nurse positions and submit the completed form, along with the application of the selected candidate, to Employee Services Agency – Human Resources for approval. This form allows the County to appoint newly hired CN or Psych Nurses to a step higher than the lowest step listed in the Basic Salary Plan. 
  • New Employee Checklist

    This form is used by supervisors to verify that everything has been completed prior to an employee starting work, and for a new employee to orient himself/herself to County employment.

  • Employee Separation Checklist

    This form is used by supervisors to verify that everything has been completed prior to an employee's last day of work.

  • Add & Delete - Admin

    An add and delete action is the County's method of changing a vacant position to that of another existing classification without going through a classification study. Administrative Add/Delete: The County Executive under certain specified conditions has the authority to approve add/delete actions when the organizational and fiscal impacts are minor.

  • Add & Delete - Formal

    An add and delete action is the County's method of changing a vacant position to that of another existing classification without going through a classification study. Formal Add/Delete: Must have full approval by the Board of Supervisors.
