If you've decided to make benefits changes for the 2024 - 2025 plan year: 

  1. You'll contact your dedicated Employee Benefits Representative to request the forms you need. Click here for your representative's direct contact information, or contact us using the information below:

  2. Return all required forms to the Employee Benefits Department by May 31, 2024.
  3. Return your forms to the Employee Benefits Department by fax or email as shown
    below — Do not send any enrollment forms directly to the medical plan carriers.

Following Open Enrollment
The effective date for your Open Enrollment elections is July 1, 2024. 

NOTE: If you change your medical plan, it’s very important that you verify coverage before seeking services. It normally takes the plan providers about 3–4 weeks to send out an official enrollment packet and new membership ID card(s).

Finally, we encourage you to take note of when changes to your benefits are allowed during the year. Refer to the online 2024–2025 Retiree Benefits Guide to see your special enrollment rights and other required Plan Notices. You can also contact your Department Employee Service Center  to request a copy of the required Plan Notices.
